REGISTERING WITH THE PRACTICE
You may register with the practice if you live within our practice area. If you are currently registered with another practice within Hatfield you will need to discuss transferring to us with either a doctor or our practice manager.
If you are currently registered with another practice outside of this area and would like register with us, please pick up a registration form from reception. When registering you will need to provide either your tenancy agreement, rent book or proof of ownership of you residence, together with 2 utility bills.
If you have never been registered with any practice in the UK you will also need to provide ID in the form of a passport. You may be asked to provide further evidence to establish your eligibility for NHS treatment.
This is a clearly defined area which includes Hatfield, Colney Heath, Smallford, Eastern St Albans, Welham Green, Brookmans Park and Southern Welwyn Garden City. You can only be registered with us if you live within this area – please ask at reception for more details.
CHANGE OF NAME OR ADDRESS
If you move or change your name or telephone number, please let us know so that we can keep your records up to date.
If you are not registered with the practice and need to be seen in an emergency you can be seen as a temporary resident. Temporary residency is valid for a period of three months.
